SALARY RANGE:               

        • Degree In Progress: $32.80/hr.
        • BBS Registered: $34.99/hr. to $36.96/hr.
        • LMFT/LPCC/LCSW: $39.17/hr. to $47.87/hr.

JOB SUMMARY: Reports directly to the BH Manager, Wraparound. Provide screening, outreach and engagement and transition services. The Wraparound Facilitator will work at all CommuniCare+OLE Clinic sites and outreach locations throughout Yolo County, to include home visits, work in schools, and juvenile detention facility. On call hours required.

EDUCATION, EXPERIENCE, TRAINING

  1. Bachelors in relevant field + 5 or more years experience required.
  2. ASW/AMFT or other relevant Master’s degree preferred
  3. A minimum of two years experience working with youth involved in the Child Welfare and/or Juvenile Justice System in behavioral health field required
  4. Previous experience providing Wraparound services preferred.
  5. Bilingual English/Spanish required
  6. Must have current/valid Driver’s License and clean driving record

 

KNOWLEDGE OF WORK

1. Understanding and use of DSM IV.
2. Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers.
3. Expected to have strong interpersonal skills and have the ability to understand and follow written/verbal instructions.

DUTIES AND RESPONSIBILITIES

1. Conducts screening for clients and their families referred through M-Dart who are involved with the Child Welfare and/or Juvenile Justice System
2. Facilitates initial strengths, needs, and cultural discovery process for families referred for Wraparound.
3. Convenes and facilitates Child and Family Team meetings with child, family, natural supports and formal supports/professionals.
4. Works with child and family to develop safety plan and individualized child and family plan.
5. Utilizes evidence-based assessment and outcome measurements tools, compiles and reports data.
6. Completes all client information and documentation in a timely manner (48 hours) according to guidelines.
7. Maintains a 65%+ level of productivity of direct client service.
8. Works with clients to establish long and short-term goals.  Clinician is responsible for detecting emotional, psychological, and physical danger signals.  Clinician is non-judgmental and offers feedback and support to clients.
9. Responsible for case management, crisis intervention, safety planning and collateral services for youth.
10. Provides information to youth and families including treatment alternatives and referral service.
11. Responsible for remaining within the ethical standards established in the field, including those of confidentiality.
12. Attend clinical supervision and weekly staff meetings.  Attend training/conferences as arranged.

Responsible for maintaining the standards as listed in the CommuniCare+OLE Personnel Policies and Procedures Manual, and any other standards which may apply to this position.
14. Works as a part of a multi-disciplinary team including Probation, Juvenile Detention Facility staff, CWS, behavioral health clinicians, Youth and Family Specialists, youth and parent partners, and other county agencies to coordinate care for youth and families as directed by supervisor.
15. Other projects as may be assigned.
