Salary Range: $52.96 to $64.24 hourly

JOB SUMMARY This position is responsible for telephone triage and clinical support services for CommuniCare+OLE patients. The RN is responsible for answering the nurse triage line, which may include, but is not limited to, performing an assessment of patient symptoms, and referring them to the appropriate level of care (home care, appointment with primary care provider (PCP), and/or escalating treatment to higher level of care like the emergency department). In addition, the RN will support the throughput of primary care services through processing prescription refills, documentation abstraction, and following-up on patients regarding their care plan. The RN will collaborate with case management, nursing teams, providers, and other departments to ensure quality patient care as needed.

EDUCATION, EXPERIENCE, TRAINING

  1. Completion of an accredited RN program.
  2. Current California RN license and in good standing with the Board of Registered Nurses required.
  3. Current CPR certification required.
  4. Previous nursing experience in a medical office, outpatient clinic or hospital.
  5. Bilingual Spanish/English preferred.

 

KNOWLEDGE OF WORK

1. Knowledge of nursing process, scope of practice and standards of care.
2. Utilizes the nursing process (assessment, planning, intervention and evaluation) in patient care and daily activities.

 

DUTIES AND RESPONSIBILITIES

1. Performs telephone triage using written triage protocols to determine patient disposition.
2. Furnishes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions, within scope of practice.
3. Provides direct one-on-one patient education.
4. Provides follow-up to patients recalled for abnormal labs/tests.
5. Transmits new, updated and refilled prescription orders to pharmacies on behalf of prescribing providers.
6. Processes clinical patient information for providers in patient electronic health records (EHR).
7. Performs other duties as assigned.
