The Perinatal Support Specialist – Welcome Baby: Road to Resilience (WB:R2R) supports in-clinioutreach and navigation for the WB:R2R program. This position meets eligible perinatal patients at CCOLE perinatal and pediatric appointments to discuss WB:R2R services and enroll them in the program; completes brief needs screening and linkage to community resources for enrolled families; coordinates outreach with other CCOLE service providers to reach hard to engage patients, including attending home visits with other WB:R2R staff; and may perform other program outreach activities at local hospitals and community events. WB:R2R is a grant funded program that focuses on family support during pregnancy and early childhood. Funding for WB:R2R is committed through June 2027 | |||
HOURS/SITE: Full time, 40 hours per week. Primary site is Salud Clinic in West Sacramento in a hybrid format, with frequent travel to Davis Community Clinic, Hansen Clinic in Woodland, client homes, community outreach, and partnering hospitals throughout Yolo County. May include some evening and weekend hours.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES (add or delete if necessary)
1. | Complete initial outreach for WB:R2R for CommuniCare+OLE patients in CommuniCare+OLE clinics and Yolo County Residents referred to WB:R2R from other sources. Educate clients on home visiting and services available through WB:R2R. |
2. | Consent clients to WB:R2R services using the WB:R2R Initial Consent. |
3. | Complete New Baby Questionnaire (NBQ) initial client needs assessment and work with WB:R2R project manager to triage clients to appropriate WB:R2R services, and/or other home visiting services, and/or other community resources as applicable. |
4. | Work closely with the WB:R2R BH Navigation Coordinator and BH Navigators to support the highest-need WB:R2R clients with outreach and engagement. Coordinate WB:R2R outreach with CommuniCare+OLE perinatal social worker as applicable. |
5. | Complete WB:R2R BH intake paperwork with clients. |
6. | Attend WB:R2R RN visits as needed to engage clients in long-term home visiting and other services. |
7. | Attend community outreach events to represent WB:R2R and attain community referrals as needed. |
8. | Coordinate with the perinatal health coordinator to round at hospitals serving Yolo County residents to identify and enroll eligible clients into the WB:R2R RN visit. |
9. | Assist WB:R2R Project Manager, Perinatal Health Coordinator, and BH Navigation Coordinator with purchasing of direct to client materials and inventory management. |
10. | Assist WB:R2R Project Manager with R2R client chart reviews and data collection activities as needed. |
11. | Complete professional documentation in a timely and comprehensive manner according to CommuniCare+OLE policies and WB:R2R contract requirements. |
12. | Attend trainings as needed to meet WB:R2R contractual obligations and for continuing education. |
13. | Attend WB:R2R and community-based meetings as needed. |
14. | Performs other duties as assigned. |
