The Integrated Behavioral Health Provider is responsible for providing quality behavioral healthcare services to patients including psychosocial assessment, intervention, referral, and consultation to providers as well as managing and tracking data.

ANNUAL SALARY of an ASW/AMFT based on 1.0 FTE:$85,072 to $94,224
ANNUAL SALARY of an LCSW/LMFT based on 1.0 FTE: $108,763.20 to $132,932.80
HOURS: 40 hours a week, Mon to Fri onsite

JOB SUMMARY: Responsible for providing quality behavioral healthcare services to patients including psychosocial assessment, intervention, referral and consultation to providers.
KNOWLEDGE OF WORK
Knowledge of Federally Qualified Health Centers and the key health center principles and guidelines |
Knowledge of the current Diagnostic Statistical Manual of Mental Disorders and ability to accurately use this knowledge to diagnose patients and inform treatment planning |
Knowledge of psychotropic medications/medically assistant treatments and physical health conditions/chronic disease |
Knowledge of crisis intervention, applicable laws and ethical consideration related to patient rights, treatment, safety and confidentiality |
DUTIES AND RESPONSIBILITIES
Provides brief behavioral health interventions for patients during their medical/dental/nutrition/pharmacy visit |
Provides behavioral health support to patients involved in the MAT program |
Collaborates with specialty behavioral health/ Alcohol and Drug Services, to ensure streamlined referrals and open communication with the specialty behavioral health/ADS providers; provide warm handoffs to county ADS/MH when appropriate |
Provides consultation to appropriate clinic staff and providers on behavioral health issues of patients |
Guides and assist clinic staff in making CPS reports or contacting law enforcement as needed |
Provides crisis intervention, follow-up visits, and appropriate referrals to other agencies in the community as needed; advocate on behalf of the patient to obtain services as necessary |
Facilitates behavioral health groups and group medical visits |
Provides education and support to patients and their families to encourage compliance with medical and behavioral regimes |
Develops strong working relationships with patients, and reach out to patients who are non-adherent or disengaged |
Helps manage/maintain database of patients receiving Suboxone/Vivitrol treatment |
Catalogues and maintains up-to-date information on addiction/Mental Health/community resources |
Documents patient visits and related information into the electronic medical record, in a timely and thorough manner |
Develops and present trainings, in-services, and updates to providers and staff on behavioral health topics |
Performs other duties as assigned |
EDUCATION, EXPERIENCE, TRAINING
- Master’s Degree in Social Sciences required.
- Current California Associate Social Work certificate or Licensed Clinical Social Worker license required.
- Must certify and remain current in CPR.
- Previous experience in a community healthcare center preferred.
- Strong verbal and written communication skills; Bilingual (English/Spanish) language preferred.
- Strong interpersonal and customer service skills required; ability to handle difficult patients.
- Computer skills, including how to conduct searches on the Internet and knowledge of Microsoft Office required; electronic health record system preferred.
- Must have strong analytical and problem-solving skills.
