PAY RANGE: $28.25 to $34.53 hourly 
JOB SUMMARY/OVERVIEW:

The Comprehensive Perinatal Health Worker (CPHW) is responsible for providing a wide range of culturally competent comprehensive perinatal services.  The CPHW conducts health screenings, initial comprehensive and trimester health assessments, provides health education, and provides other health services as assigned. The CPHW promotes positive behavior change and advocates for healthful pregnancy and postpartum outcomes for pregnant patients.  The CPHW provides services to patients from conception through the postpartum period, based on the State of California Medi-Cal Comprehensive Perinatal Services Program (CPSP) guidelines and protocols.     The CPHW is part of an interdisciplinary team that includes the Midwife/Medical Provider, Medical Assistant, Registered Nurse, Registered Dietitian Nutritionist, Lactation Consultant, and Social Worker.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING

  1. Education:
  • High school graduate or equivalent required.
  • Bachelor’s degree in related field preferred.
  1. Experience/Lived Experience:
  • At least one year of full-time paid practical experience in prenatal, postpartum, or women’s health services, required.
  • Experience providing health education in a healthcare setting preferred.
  1. Special Skills/Training:
  • Have or obtain Comprehensive Perinatal Services Program (CPSP) Steps to Take Training Certificate.
  • Current BLS for Healthcare Providers certificate required.
  • Bilingual (English/Spanish) strongly preferred.
  • Computer skills and knowledge of Microsoft Office required; electronic health record system experience preferred.

 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES 

1. Comprehensive Perinatal Services Program (CPSP) Scope of Services includes:

  1. Provide a CPSP Orientation to orient the client to comprehensive perinatal care at entry of care and through the pregnancy.
  2. Complete a CPSP Initial Assessment for the three components of Nutrition, Health Education, and Psychosocial using approved documentation, forms and protocols.
  3. Develop and complete a CPSP Individualized Care Plan (ICP) and continually revise and update the ICP as needed.
  4. Provide a CPSP-approved Interventions such as services, classes, counseling, referrals, and instructions as appropriate for the patient’s needs and risks as identified on the ICP.
  5. Provide a CPSP Reassessment in each trimester to identify changes or new developments since the previous Assessment and provide continued support.
  6. Provide a CPSP Postpartum Assessment of the mother and infant and update the ICP with appropriate interventions for the patient and her baby.
2. Provide individual and group education on a variety of topics, including, but not limited to, breastfeeding, newborn care, and childbirth preparation.  Develop and implement curricula for classes as needed.
3. Facilitate referrals to Midwives/Medical Providers, Registered Dietitian Nutritionists, Social Workers, Lactation Consultants, Dentists, and other appropriate services as necessary.
4. Facilitate external community referrals to appropriate perinatal support services (i.e. WIC, etc.) as necessary.
5. Screen for and issue temporary Medi-Cal (Presumptive Eligibility); follow up with patients until permanent Medi-Cal has been issued.
6. Complete CPSP-required documentation, including billing information, according to protocol for all patient encounters (face-to-face and telephonic) and activities in the in the patient’s electronic health record.
7. Assist with data entry, data tracking, review and closure of CPSP patient charts.
8. Check work email and organizational communications daily.
9. Participate in community-based organizations and groups as assigned.
10. Represent CommuniCare+OLE Perinatal Services at community events as assigned.
11. Attend all in-service and trainings as assigned.
12. Performs other duties as assigned.
