Established in 2023, CommuniCare+OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

 

We are looking to hire an Advanced Practice Provider (Physician Assistant or a Family Nurse Practitioner) for CommuniCare+OLE!

SUPERVISION GIVEN:     None

FULL TIME EQUIVALENT: 0.8 to 1.0 FTE

NP/PA Annual Salary: $157,684.80 to $192,712.00 (Based on a 40 hour work week)

 

JOB SUMMARY: Responsible for providing quality primary health care to patients while working closely with other healthcare staff in a team approach.

 

KNOWLEDGE OF WORK

Knowledge of Electronic Health Records and related computer systems
Ability to collaborate with mutual providers of treatment in mental health and substance use settings as well as work well with the primary care team
Ability to prioritize and multi-task and have strong time management skills

DUTIES AND RESPONSIBILITIES

Provide the following health care services for patients: assessment/evaluation, diagnosis, treatment/prescribing, planning and education.  Consult with staff physicians as needed
Review and manage patient information including labs, diagnostic images, prescription refills, outside consultation notes, previous medical records, specialist referrals, etc.
Prescribe and administer medications and treatment regimens based on assessed medical conditions.  Review medication(s) that patient is currently taking, prior to prescribing to prevent adverse reactions
Refer patients to specialists and to relevant patient care services as appropriate
Prioritize patient care throughout the day
Provide acute triage over the phone or with walk in patients
Provide education and information to patients on various issues, providing handouts as appropriate
Perform various in office procedures as directed
Document all appropriate information in electronic health record system
Respond to messages and relay information to patients and any other internal or external sources as needed
Complete various medical related forms as needed
Direct and coordinate the patient care activities of nursing and support staff as appropriate
Research on unusual cases
May participate in and collaborate with staff/teams on various support-related functions including quality assurance, care team development, system optimization, empanelment, policy and procedure development, etc.
Assists in the coordination and integration of all health clinic outreach programs as directed
Participate in education of students
Perform other duties as assigned

 

EDUCATION, EXPERIENCE, TRAINING FOR PA

  1. Graduate of a Physician Assistant program from an accredited school required
  2. Current California Physician Assistant License required
  3. National Commission on the Certification of Physician Assistants Certificate required
  4. Must have or obtain current California medication furnishing license
  5. Must have or obtain current DEA License
  6. Current Basic Life Support CPR certification required
  7. Experience in a community healthcare center preferred
  8. Strong verbal and written communication skills; Bilingual (English/Spanish) language required
  9. Strong interpersonal and customer service skills required; must have ability to handle difficult patients
  10. Must have strong time management skills, ability to prioritize, and work in a fast-paced environment
  11. Excellent problem-solving skills required
  12. Computer and data entry skills required; electronic health record system preferred

  1. Graduate of an accredited Master of Science in Nursing required.
  2. Current California Nurse Practitioner certification required.
  3. Must have or obtain current California medication furnishing license.
  4. Must have or obtain current DEA License.
  5. Must certify and remain current in CPR certification.
  6. Experience in a community healthcare center preferred.
  7. Strong verbal and written communication skills; Bilingual (English/Spanish) language required.
  8. Excellent interpersonal and customer service skills required; must have ability to handle difficult patients.
  9. Must have strong time management skills, ability to prioritize, and work in a fast-paced environment.
